spasm.<\/span><\/li><li><span class=\"n\">07<\/span><span>Pain that affected sleep.<\/span><\/li><\/ol>\n<\/div>\n            <p>The low back and tailbone pain became more noticeable and worsened in the days after the collision.<\/p>\n          <\/section>\n          <section id=\"gap\" aria-labelledby=\"gap-title\">\n            <span class=\"eyebrow\"><span class=\"num\">05<\/span>Treatment Gap and Pain Management<\/span>\n            <h2 id=\"gap-title\">Treatment Gap and <span class=\"accent\">Pain Management<\/span><\/h2>\n            <p>The client did not begin formal pain management treatment immediately after the August 15 crash.<\/p>\n            <p>She began treatment with a pain management provider on <time datetime=\"2024-09-04\">September 4, 2024<\/time>.<\/p>\n<div class=\"gapline\"><div class=\"gp\"><span class=\"gd\">August 15, 2024<\/span><span class=\"gt\">Rear end crash<\/span><\/div><div class=\"gmid\"><span class=\"gl\">Treatment gap<\/span><\/div><div class=\"gp end\"><span class=\"gd\">September 4, 2024<\/span><span class=\"gt\">Pain management treatment begins<\/span><\/div><\/div>\n            <p>The existence of a treatment gap did not change the core liability evidence: she was stopped in traffic when another driver failed to control speed and rear ended her.<\/p>\n            <p>It also did not prevent the legal team from building a  <p>The client continued with chiropractic care from late September through mid November 2024.<\/p>\n            <p>She attended approximately 20 chiropractic visits.<\/p>\n            <p>Treatment included:<\/p>\n<ul class=\"tiles5\" role=\"list\"><li>Electrical stimulation.<\/li><li>Mechanical traction.<\/li><li>Spinal adjustments.<\/li><li>Massage.<\/li><li>Therapeutic exercise.<\/li><\/ul>\n            <p>Her chiropractic diagnoses included lumbar sprain, myalgia, and sleep disorder.<\/p>\n<ol class=\"stages\" role=\"list\"><li><span class=\"sl\">Initial chiropractic examination<\/span><span class=\"st\">At the initial chiropractic examination, her lumbar range of motion was moderately restricted.<\/span><\/li><li><span class=\"sl\">Mid treatment reevaluation<\/span><span class=\"st\">At a mid treatment reevaluation, her progress was described as slower than expected.<\/span><\/li><li><span class=\"sl\">November reevaluation<\/span><span class=\"st\">By the November reevaluation, her progress was described as improving as expected. Her pain was intermittent, and she did not report numbness, sleep problems, or medication use at that visit.<\/span><\/li><\/ol>\n          <\/section>\n          <section id=\"mri\" aria-labelledby=\"mri-title\">\n            <span class=\"eyebrow\"><span class=\"num\">07<\/span>MRI Findings<\/span>\n            <h2 id=\"mri-title\">MRI <span class=\"accent\">Findings<\/span><\/h2>\n            <p>A lumbar MRI performed on <time datetime=\"2024-09-12\">September 12, 2024<\/time>, documented findings at L4-5 and L5-S1.<\/p>\n<div class=\"vcard\"><p class=\"vsub\">At L4-5, the MRI identified:<\/p><ol class=\"vlist\" role=\"list\"><li><span class=\"n\">01<\/span><span>A 3 mm disc bulge with bone spurs.<\/span><\/li><li><span class=\"n\">02<\/span><span>A subtle 2 mm central disc herniation.<\/span><\/li><li><span class=\"n\">03<\/span><span>An annular tear.<\/span><\/li><li><span class=\"n\">04<\/span><span>Mild to moderate narrowing of the right side nerve opening.<\/span><\/li><\/ol>\n<\/div>\n<div class=\"vcard\"><p class=\"vsub\">At L5-S1, the MRI identified:<\/p><ol class=\"vlist\" role=\"list\"><li><span class=\"n\">01<\/span><span>A 3.5 mm disc bulge with bone spurs.<\/span><\/li><li><span class=\"n\">02<\/span><span>A 3 mm right sided disc herniation.<\/span><\/li><li><span class=\"n\">03<\/span><span>An annular tear.<\/span><\/li><li><span class=\"n\">04<\/span><span>Mild narrowing of the spinal canal and right side nerve pathway.<\/span><\/li><li><span class=\"n\">05<\/span><span>Moderate disc degeneration and endplate changes.<\/span><\/li><\/ol>\n<\/div>\n<p class=\"fyi\">The records describe certain findings (including bone spurs, loss of disc height and hydration, and endplate changes) as degenerative. The client&#39;s medical providers nevertheless evaluated the MRI in connection with her post crash low back symptoms and treated her for lumbar disc herniation and radiculopathy.<\/p>\n          <\/section>\n          <section id=\"injection\" aria-labelledby=\"injection-title\">\n            <span class=\"eyebrow\"><span class=\"num\">08<\/span>Spinal Injection<\/span>\n            <h2 id=\"injection-title\">Spinal <span class=\"accent\">Injection<\/span><\/h2>\n            <p>Conservative treatment alone did not immediately resolve the client&#39;s symptoms.<\/p>\n            <p>As treatment continued, the client elected to proceed with an interventional spinal procedure.<\/p>\n<div class=\"record\"><div class=\"rh\"><span class=\"rt\"><time datetime=\"2024-10-23\">October 23, 2024<\/time><\/span><\/div><div class=\"rlead\"><p>On October 23, 2024, she underwent right sided spinal injections at L4-5, L5-S1, and S1 under IV anesthesia.<\/p><p>The procedure included right sided nerve root blocks and disc nerve blocks.<\/p><p>The procedure diagnosis included lumbar disc herniation and radiculopathy.<\/p><\/div><\/div>\n<div class=\"pain\"><div class=\"ph\">Reported low back pain<\/div><div class=\"pr\"><span class=\"pk\">First pain management examination<\/span><span class=\"pb\" aria-hidden=\"true\"><span style=\"width:80%\"><\/span><\/span><span class=\"pv\">8 out of 10<\/span><\/div><div class=\"pr\"><span class=\"pk\">November 1 follow up<\/span><span class=\"pb\" aria-hidden=\"true\"><span style=\"width:30%\"><\/span><\/span><span class=\"pv\">3 out of 10<\/span><p class=\"pt\">At the November 1 follow up appointment, the client reported that her pain had greatly improved and was at a 3 out of 10 level, mainly in the morning. She reported that she had not needed medication since the injection.<\/p><\/div><div class=\"pr\"><span class=\"pk\">November 15, 2024<\/span><span class=\"pb\" aria-hidden=\"true\"><span style=\"width:0%\"><\/span><\/span><span class=\"pv\">No low back pain<\/span><p class=\"pt\">At her final pain management appointment on November 15, 2024, she reported no low back pain, planned to stop chiropractic treatment, and was discharged from pain management.<\/p><\/div><\/div>\n          <\/section>\n          <section id=\"treatment\" aria-labelledby=\"treatment-title\">\n            <span class=\"eyebrow\"><span class=\"num\">09<\/span>Treatment and Medical Documentation<\/span>\n            <h2 id=\"treatment-title\">Treatment and <span class=\"accent\">Medical Documentation<\/span><\/h2>\n            <p>After the rear end collision, the client&#39;s low back and tailbone pain continued and required a structured course of care. She began with pain management treatment, underwent diagnostic imaging, completed chiropractic care, and later proceeded with an interventional spinal procedure after conservative treatment did not immediately resolve her symptoms.<\/p>\n            <p>Her records documented evaluation for lumbar sprain and strain, muscle spasm, radiating symptoms, and lumbar disc related findings. The treatment plan included medication, MRI imaging, chiropractic care, follow up appointments, and a spinal injection. After the injection, the client reported substantial improvement.
